Detection via culture or directly from clinical specimens

The emergence and spread of multidrug-resistant and extensively drug-resistant Mycobacterium tuberculosis strains pose a serious threat to tuberculosis control. Multidrug resistance (MDR) is defined as resistance to at least two of the most effective first-line drugs, isoniazid (INH) and rifampicin (RIF).

In extensively drug-resistant TB (XDR-TB), M. tuberculosis strains are resistant not only to RIF and INH, but also to at least one fluoroquinolone (FQ) and one of the injectable second-line antibiotics amikacin, kanamycin, and capreomycin.

Treating resistant TB is difficult because it causes more severe side effects, the patient remains infectious for longer, and it is significantly more expensive. Rapid resistance testing is essential to initiate effective treatment in a timely manner, as pathogen detection via culture takes several weeks.
