In cases of classic whooping cough symptoms, the diagnosis is often made based on clinical findings. Further diagnostic testing is indicated in cases of prolonged cough without typical coughing fits, particularly in unvaccinated children, adolescents, and adults, but also in vaccinated individuals.

In the early stages of primary infection, especially in infants, direct detection of Bordetella pertussis and Bordetella parapertussis from deep nasopharyngeal swabs or nasopharyngeal secretions and similar material is possible.

The culture method previously used as the gold standard requires considerable expertise in specimen collection and transport. In addition, culturing Bordetella pertussis takes at least three days.
