Hyperhomocysteinemia is a risk factor for coronary artery disease and venous thrombosis.

Homocysteine metabolism occurs via two metabolic pathways. Genetic defects in the genes encoding the enzymes involved lead to severe or milder forms of hyperhomocysteinemia, depending on the type of mutation. One of these metabolic pathways involves the enzyme methylenetetrahydrofolate reductase (MTHFR), which catalyzes the conversion of homocysteine to methionine.

The test examines the C677T and A1298C polymorphisms in the MTHFR gene, which result in reduced enzyme activity and are associated with coronary artery disease and venous thrombosis.
